Your first visit

Thank you for trusting us to be a part of your health journey! We typically schedule 60 minutes for your first appointment. Please plan to arrive 10 minutes prior to your appointment to complete your new patient registration. Better yet, complete the form online prior to your arrival. See below.

New patient registration

We kindly ask that you submit your new patient registration form, complete with your medical history and form of ID, prior to your appointment. This is a one-time process and required by the DHA in order to be seen by your medical professional. Valid also for new biophysio, integrative medicine, and homeopathy patients.

Insurance policy

We operate on a pay-and-claim basis. Here’s how it works:

  1. Treatment payment: You’ll be asked to pay the full amount at the time of treatment. Payment plans are available on a case-by-case basis, to be discussed with our treatment coordinator.
  2. Claim submission: After your treatment, you’ll receive an insurance claim form, detailed invoice, and proof of payment. Submit these documents to your insurance provider.
  3. Reimbursement: Your insurance provider will process your claim and reimburse you directly, depending on your coverage. If they request additional information, please contact our clinic.

Cancellation policies

We understand that life can be unpredictable, and plans may change. If you need to cancel or reschedule your appointment, please do so at least 24 hours in advance. This allows us to offer the slot to another patient in need. Frequent late cancellations or missed appointments may incur a fee. We appreciate your understanding and cooperation in helping us provide timely care for all our patients.
